- Physical Exam Record
- A Certified Birth Certificate
- Copy of Health Insurance Card
- Copy of Parent(s) or Guardian’s ID/ Driver’s License
- School Record from previous school or Transcript, if applicable
- Copy of IEP (Individualized Educative Plan if applies)
- Recommendation Letter
